Correction of Date/Place of birth involving Issuing New Passport

• Online passport application form.
• 2 recent clear photos 51mm*51mm (not older than three months, no uniform) with white background and DARK DRESS with frontal view. People with White hair/white beard/religious white cap should submit photos with Light blue background. Ears, eyes open ,forehead to chin to be completely visible, no shadows on the face and neck, no reflections on the glasses.
• Change of appearance Sworn Affidavit is required if (Hair to Bald headed and vice-versa Turban to Non-Turban and vice-versa, Scarf to No-Scarf and vice-versa etc.)
• Signature/ Thumb impression should be in DARK BLACK INK and should not touch the lines of the box.
• The signature needs to be done in presence of submission officer.
• Applicant has to put the signature same as per current passport.If changing the signature then Sworn Affidavit required. Minors can change the signature up to 18 years without any supporting documents.
• Current Passport in original with photocopy (of first, last, address page (if separate page), any other endorsement pages, Valid UAE visa page) and additional booklets Original, if any. For minors (up to 18 years) parents' passport copies (of first, last, address page (if separate page), any other endorsement pages, Valid UAE visa page) are required.
• If Visa is expired and in grace period, those who are under same Company Sponsorship photo testified letter from the company (in English) and valid trade license copy is required. For dependants Sponsor's Photo testified letter and passport copy is required. In case if visa is expired and crossed the grace period prior approval required from CGI Dubai along with instruction note from BLS.
• For new employment - Offer Letter attested by Chamber of Commerce (Dubai/Sharjah & Northern Emirates) where the company is registered OR Valid employment Paper VISA in Original and copy, photo testified letter from the company (in English) and valid trade license copy is required.
• A passport expired for more than 6 months up-to 3 years should be supported by a Sworn Affidavit explaining the circumstances and the reasons for late renewal. If expired more than 3 years and to 5 years Sworn Affidavit as per Annexure "I" along with approval from Consulate.
• A declaratory Court order (in original with a copy) issued by a First Class Judicial Magistrate in India instructing the Consulate to correct Date/Place of Birth.
• Sworn Affidavit, in the prescribed format, signed by the applicant in the presence of the consular officer for Correction of Date of birth and Place of birth and prior approval required from CGI Dubai along with instruction note from BLS.
• Photo ID card copy with expiry details of ID - Driving License, Emirates Id Card, Health Card, Labour Card,School Identity card in case of Minors.
• The applicant should submit the application in person for identification. But in case of minors BOTH PARENT has to accompany the child.
• Any Changes/Corrections to be amended in the passport should be accompanied with documentary evidence,Sworn Affidavit and prior approval required from CGI Dubai along with instruction note from BLS and the same will take Minimum 60 working days.
• Applicants holding passports in which Place of Issue, Place of Birth or Permanent Address is from J & K or Srinagar, Personal Particulars form in Triplicate with photo pasted on each of them is required along with application. Processing time will be Minimum 60 Working Days and no TATKAL Service.

Note: Abu Dhabi/Al-Ain visa holders applying at centre in Dubai/Northern Emirates should provide the address proof in their name for the residency in Dubai/Northern Emirates i.e. Tenancy Contract/Electricity Bill in English. For Dependants photo testified letter from the sponsor with the details of the accommodation mentioned and tenancy contract in the sponsor's name along with passport copy of the sponsor is required. If residing in company's accommodation then photo testified letter from the company (in English) on Company Letter Head stating that their Visa is from Abu Dhabi/Al-Ain but currently working and residing in Dubai/Northern Emirates.

• Dubai Issued – Minimum 5 Working Days
• Other PIA (Adults) – Minimum 5 Working Days
• Other PIA (Minors) – Minimum 5 Working Days
• TATKAL - Minimum 2 Working Days

Processing time and validity of the passport is under the sole discretion of CGI DUBAI
