New passport for change of name of other than above cases

  • Application form EAP – I (External). All particulars in the application should tally with those in the present passport.
  • Personal Particulars Form (in triplicate with photo pasted on each of them).
  • 6 recent clear photos 51mm*51mm (not older than three months, no uniform) with white background and DARK DRESS with frontal view. Ears, eyes open, forehead to chin to be completely visible, no shadows on the face and neck, no reflections on the glasses.
  • Signature/Thumb impression should be in DARK BLACK INK and should not touch the lines of the box.
  • The signature needs to be done in presence of submission officer.
  • Applicant has to sign same as per current passport. If changing the signature, request letter addressing to consulate general of India to be submitted.
  • Current Passport in original with clear photocopy (of first, last, address page (if separate page), any other endorsement pages, Valid UAE visa page) and additional booklets Original, if any.
  • For minors (up to 18 years) passport original for verification and copies are required.
  • For full name change - application can be accepted with self declaration (in the same format of affidavit).
  • At least two public/school documents or Valid Indian ID issued in the desired/applied changed name.
  • Newspaper advertisement in original, of two leading daily newspapers, notifying the change of name. One daily newspaper should be of the area of applicant's permanent address in India and another daily newspaper of the area of applicant's present address in UAE. Please attach full sheets of the newspaper and not cuttings thereof. OR Original Gazette notification of the concerned state government.
  • Both News paper advertisements should complete 30 days of notification period.

*Format of newspaper notification for MINOR is given below:-
I,________________father of______________, holder of Indian Passport No_____________, issued at________on________, permanent resident of____________(full postal address in India to be given) and presently residing at____ (full postal address in UAE to be given) do hereby change my Son/daughter name from ______to____, with immediate effect.

  • Presence of applicant while submitting application is compulsory to establish identity. But in case of minors both parent has to accompany the child with parents original passport.
  • Photo ID card copy (Driving License, Emirates Id Card, Health Card, Labour Card, Election Commission card School Identity card in case of Minors...etc).
  • If Visa is expired and in grace period, those who are under same Company, Sponsorship photo testified letter from the company (in English) and valid trade license copy is required. For dependants Sponsor's Photo testified letter and passport copy (with valid UAE Visa page) is required. In case if visa is expired and crossed the grace period prior approval required from Consulate General of India, Dubai along with instruction note from BLS.
  • For new employment - Offer Letter attested by Chamber of Commerce (Dubai/ Sharjah & Northern Emirates) where the company is registered OR Valid employment Paper VISA in Original and copy, photo testified letter from the company (in English) and valid trade license copy is required.
  • Indian Female citizen married to GCC nationals and holding Short Validity Passport (SVP), pre-approval is required. Applicant is required to visit personally to the consulate for pre-approval.
  • In case of female applicant if the visa status is showing as HOUSEWIFE and name of sponsor is not matching with husband name in the passport,then NOC & passport copy of the sponsor is required.
  • A Passport expired for more than 6 months up-to 1 year should be supported by a request letter explaining the circumstances and the reasons for late renewal. If passport expired for more than 1 year then request letter with proper explanation and prior approval required from Consulate General of India, Dubai along with instruction note from BLS.
  • Any Changes/Corrections to be amended in the passport should be accompanied with documentary evidence; request letter with proper explanation and prior approval required from Consulate General of India, Dubai along with instruction note from BLS
  • Applicants holding passports in which Place of Issue, Place of Birth or Permanent Address is from J & K or Srinagar, Personal Particulars form in Triplicate with photo pasted on each of them is required along with application. Processing time will be Minimum 5 Working Days and no TATKAL Service.

Note : Abu Dhabi/Al-Ain visa holders applying at centre in Dubai/Northern Emirates should provide the address proof in their name for the residency in Dubai/Northern Emirates i.e. Tenancy Contract/Electricity Bill. For Dependants photo testified letter from the sponsor with the details of the accommodation mentioned and tenancy contract in the sponsor's name along with passport copy of the sponsor is required. If residing in company's accommodation then photo testified letter from the company (in English) on company letter head stating that their Visa is from Abu Dhabi but currently working and residing in Dubai/Northern Emirates.

Processing time and validity of the passport is under the sole discretion of Consulate General of India, DUBAI
